This policy explains what information D2H Wellness collects when you visit www.d2hwellness.com, book a session, complete our intake form, or use our client portal and mobility app, how we use it, and the choices you have.
1. Information we collect
Information you give us. Your name, email address, phone number, date of birth, emergency contact, and, if you are under 18, your parent or guardian's information. On our intake and check-in forms we also collect health information relevant to your session, such as injuries, medical conditions, medications, areas of pain, and your goals. We keep your signed waiver and consent, and any notes your therapist writes about your sessions (SOAP notes).
Booking and payment information. Appointments and payments are handled through our scheduling and payment provider, Square. Square collects your payment card details. We do not see or store your full card number.
Client portal and mobility app. If you sign in to our client portal, we store your account email, your plan, your assigned exercises and videos, and your messages with us. Clients can see their own plan, videos and messages. Therapist session notes are never shown to clients.
Information collected automatically. Like most websites, our site and its hosting provider may collect your IP address, browser type, device, pages viewed and the date and time of your visit. Our site may display embedded content from third parties such as Google (reviews), Facebook and Instagram. Those services may set their own cookies and collect information under their own privacy policies.
2. How we use your information
- To schedule and provide massage, mobility and wellness services safely, including deciding whether a technique is appropriate for you.
- To complete required intake, waiver and consent paperwork before any session starts.
- To communicate with you about appointments, your plan, and questions you send us.
- To process payments and keep business and tax records.
- To run, secure and improve our website and apps.
- To send news or offers only if you have opted in. You can opt out at any time.
We do not sell your personal information.
3. Who can see your information
- Our therapists and staff. All of our therapists can see client records and session notes so that your care stays consistent. Access is limited to people who need it to serve you.
- Service providers that help us run the business, such as Square (booking and payments), Supabase (secure cloud storage for our client portal and app), Netlify and Replit (website and app hosting), and email providers. They may use your information only to provide services to us.
- Legal and safety. We may disclose information if required by law, to respond to a valid legal request, to protect someone's safety, or in connection with a sale of the business.
We do not share your health information for advertising.
4. Health information
Information you give us about your health is used only to provide and document your care and to meet our legal and business obligations. We take reasonable steps to protect it. This policy does not replace any separate consent or waiver you sign with us.
5. How long we keep information
We keep client records for as long as needed to provide services and to meet legal, tax and professional record-keeping requirements. When records are no longer needed, we delete or de-identify them.
6. Security
We use reasonable administrative, technical and physical safeguards, including password-protected accounts and encrypted connections. No method of storage or transmission is completely secure, so we cannot guarantee absolute security. Please keep your sign-in details private.
7. Your choices and rights
You can ask us to see, correct, or delete the personal information we hold about you, or to stop marketing messages, by contacting us at info@daretohabit.com or (845) 874-0113. We may need to keep some records where the law requires it. You can also control cookies through your browser settings.
8. Children
Our services are for adults and for minors with a parent or guardian's consent. If a client is under 18, a parent or guardian must complete the intake. We do not knowingly collect information from children online without a parent or guardian's involvement.
9. Links to other sites
Our site links to services we do not control, such as Square, Instagram, Facebook and Google. Their privacy practices are their own.
10. Changes to this policy
We may update this policy from time to time. The effective date at the top shows the latest version. Material changes will be posted on this page.
